About the program
The HOPE (Hospital Outreach suicide Postvention Engagement) Wellbeing Support Service is a suicide after-care service that has been developed to care for people discharged from hospital after a suicide attempt or crisis. The HOPE Wellbeing support service endeavours to prevent deaths by suicide by providing care coordination and supporting participants to link with supports that help keep them safe and support recovery.
About the opportunity
Under the support and direction of the Team Leader, the role forms part of the Gippsland team and works with the team, the HOPE program (Latrobe Regional Hospital) and other key stakeholders in the ongoing development and effective implementation of the Gippsland regional and program plans.
Amongst other duties, Support Coordinators provide support during the first three months following a suicide attempt while working collaboratively with existing services (clinical and community based) to meet individual needs and circumstances.
To be successful in this role
- Minimum Diploma qualifications and experience in working in a support role
for people with mental health conditions - Current valid Driver’s License and the ability to undertake travel for the role
- Current Working with Children Check and NDIS Workers Screening Check
- Right to Work within Australia
- Satisfactory National Police Records Check
- NDIS Worker Orientation Module Certificate

Job Description Summary
HOPE Wellbeing Support Service – Support Coordinator is to provide non-clinical care and practical supports to individuals in the first three months following a suicide attempt.
In particular, Support Coordinators will:
- Provide support in the first three months following a suicide attempt to help an individual to stay safe and connect with essential services
- Engage with and deliver safe and effective service to vulnerable groups who are known to be at high risk of suicide
- Reduce burden on supporting networks, including family and carers by supporting individuals to self-manage distress and increase social connectedness
- Work collaboratively with existing services (clinical and community based) to meet individual needs and circumstances.
