About the program
The Home and Community Care Program For Younger People (HACC PYP) supports younger people with disabilities or medical/health conditions, and their respective carers to live as independently as possible in their own homes. Younger people are defined as people less than 65 years, or Aboriginal and Torres Strait Islander people aged less than 50 years. The Program assists with a range of daily activities including personal care, dressing, preparing meals, house cleaning, property maintenances, community access and using public transport.
Your new role
As an Assessment and Facilitation Officer, you will complete holistic assessments of need to support the development of goal directed support plans. The role will utilitse a strengths-based approach and work in partnership with other service providers, agencies and professionals to ensure that individual clients needs can be met. The Assessment and Facilitation Officer will also lead and participate in community development and social inclusion activities. Your skills and experience
You will bring with you:
- Tertiary qualification in Social Sciences, Disability Studies or Allied Health professions, or equivalent qualifications with demonstrated industry experience.
- Current First Aid and CPR Certificate Level 2
- Current Victorian drivers licence
- Demonstrated ability to provide safe environments for children and young people and protect them from abuse and neglect.
- Knowledge of principles of community inclusion and participation, with experience in delivering a response or project as a result to a community need or service gap.
- Demonstrated experience in performing community and/or health assessments using a strengths-based approach, and development of associated care plans.
