Social Worker Grade 2
Permanent Part Time - 48 hours per fortnight
Position Summary
The Grade 2 Social Worker delivers high level clinical services across the inpatient wards and exercises professional judgement to a complex caseload, adapting routine clinical practice based on valid and reliable evidence. The successful applicant will have strong social work experience to support patients and families at Bass Coast Health and use their skills to undertake multi-dimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate complex intervention and discharge plans.
Key Responsibilities:
- Work constructively and co-operatively within a multidisciplinary health setting, the Social Work team and with a wide range of external service providers
- Engage with patients and their families to provide psychosocial assessment, discharge planning, emotional support, advanced care planning, residential aged care processes, NDIS, and guardianship & administration (VCAT) within agreed timelines
- Liaise with other multi-disciplinary staff to co-ordinate, facilitate and communicate patient treatment and discharge planning
- Liaise with external agencies to ensure effective and efficient discharge planning and managed care
- Maintain adequate records of clinical practice in line with NSQHS guidelines and BCH clinical documentation policy
- Provide nominated staff with a high standard of clinical supervision in accordance with the profession’s competency-based minimum standards (AASW standards)
