About this Program
Mental Health in Aged Care (MHiAC) supports in-reach services into Residential Aged Care Facilities (RACFs) in Gippsland. The program supports residents of RACFs who experience mild to moderate symptoms of mental illness (e.g. depression, anxiety) or who are at risk of experiencing a mental illness.
Developed by Wellways, the program provides both face to face in-reach services and telehealth supports to residents within RACFs. Mental Health in Aged Care will compliment current RACF’s programs and services through the addition of specific mental health psychoeducation programs, the development of peer leaders to support mental health outcomes, and the links to family support and education.
About the Opportunity
Under the general direction of the Senior Mental Health & Wellbeing Practitioner (MHiAC), the Mental Health & Wellbeing Practitioner will be responsible for providing wellbeing sessions with our participants in Residential Aged Care Facilites. The practitioner will maintain a caseload while also supporting clients on a waitlist, acting as the first point of contact and providing active waitlist management strategies to ensure individuals remain connected to support while awaiting services.
The role also focuses on strengthening partnerships and building collaborative links between clinical and community organisations delivering mental health services. The Mental Health & Wellbeing Practitioner will develop and maintain positive relationships with key stakeholders, including Residential Aged Care Facilities and Gippsland Primary Health Network (GPHN), to embed the service model within the broader service system and promote the program across the region. In addition, the practitioner will ensure all relevant client information and data are accurately collected and recorded to support effective service delivery and reporting.
